The issue here concerns the OPTIMUS class of SSD drives. It was found that OPTIMUS drives with firmware version 3P01 which had not been formatted after upgrading to 3P01

are susceptible to an issue involving changes to an LBA which was followed by a power cycle of the drive.

Firmware 3P02 and later addresses this issue.

If you have loaded firmware 3P01 or later on an OPTIMUS drive, you MUST reformat the drive. However, the drive MUST be VACATED prior to re-formatting.

The reformat is necessary to correct any of LBA issues that may still exist on the drive. Please verify whether or not the drive had been re-formatted.
