FI00122 indicates that a reserved IOA port on the IOP is the failing item.

If the IOP is type 2624, the failing item is type 6146 IOA.

Note the IOA type and refer to the Managing PCI Adapters document, order number SA76-0092 to determine the FRU part number to replace.
