Program Manager Disappears When Browsing a Corrupt Icon File

SYMPTOMS
In Program Manager, when you change a program item icon by choosing the
Change Icon button in the Program Item Properties dialog box, if the file
you browse for icons is corrupted, Program Manager disappears and pressing
CTRL + ESC will not bring up the task list. You are still able to close
other applications before restarting Windows NT.
STATUS
Microsoft has confirmed this to be a problem in Windows NT and Windows NT
Advanced Server version 3.1. This problem has been corrected in the latest
U.S. Service Pack for Windows NT and Windows NT Advanced Server version
